Silver & Fit Premium - Please visit contact Silver & Fit to upgrade to the premium network, then submit the form below. S&F Premium grants a PLUS Membership at Ballard Health Club.

Renew Active - Members receive a Basic Membership and may upgrade to the PLUS Membership for $30/month or $300/year

OnePass - Members receive a Basic Membership and may upgrade to the PLUS Membership for $30/month or $300/year.

FitOn - Please visit FitOn’s website to sign up for membership at BHC. Once we receive confirmation of your FitOn Membership, we will send a follow-up regarding the next steps. FitOn grants a PLUS Membership at Ballard Health Club.

To start your Silver & Fit Premium, Renew Active or OnePass Membership:

1) Create a ‘BHC Account’ account using your email address. Please add a payment method to your account if you’d like the PLUS Membership upgrade! (Renew Active and OnePass only)

2) Fill out and submit the form below. You will be contacted once your Program ID has been verified and your membership activated.

Please note that it can take up to 7 days to verify and activate your Insurance Provided Membership.

To start your FitOn Membership:

1) Create a ‘BHC Account’ account using your email address.

2) Create a ‘FitOn’ profile and link your Medicare benefits at: https://www.fitonhealth.com/members/medicare

3) Visit BHC’s page on FitOn to purchase your membership with your credits: https://explore.fitonhealth.com/in-person/location/11393
